Student Transcripts 

HOW TO OBTAIN YOUR STUDENT TRANSCRIPT  

Students whose secondary school education was completed at a Toronto Catholic District School Board secondary school may obtain an official Transcript*  as follows:

*(N.B.  an official transcript contains courses, year completed, marks achieved and whether or not the student achieved the Ontario Secondary School Diploma) 

• Former student, who last attended secondary school prior to June 2000 may apply for Official Transcript by printing a copy of the attached application form (Transcript Request Form) and mailing the completed form and fee as outlined on the form. 

• payment for transcripts must be in the form of certified cheque, money order or bank draft.  If you submit any other form of payment, your request form will be returned to you.

INDIVIDUAL TRANSCRIPT REQUESTS REQUIRE AN AVERAGE OF 5 TO 7 BUSINESS DAYS TO PROCESS.  Transcript Request Forms are processed on a “first come first served” basis 

If you have any questions or experience difficulty with printing of this form you may call the TCDSB, Archives & Records Management Dept., (416) 222-8282, 2504 for further assistance. 

• Please contact the respective secondary school which you last attended and make arrangements for the production and pick-up of transcript(s).  PLEASE NOTE, SECONDARY SCHOOLS ARE CLOSED DURING THE SUMMER MONTHS
• SCHOOLS MAY CHARGE SIMILAR FEES FOR THIS SERVICE.

EXCEPTIONS: 

1.     Students whose Toronto Catholic District School Board secondary school has closed permanently  must obtain transcript(s) from the TCDSB Archives & Records Management  Dept. 

2.    Students  who transferred to another school outside of the Toronto Catholic District School Board in order to continue with their  secondary school education must contact that particular school and/or school board in order to obtain an Official Transcript(s)
