COVID-19 Screening Passport for Students


To confirm students have passed their daily screening at home, students will be expected to present their COVID-19 Screening Passport each school day prior to entry.  Passports must be signed off by parents each day.


Use of the COVID-19 Screening Passport

  1. Parent performs daily COVID-19 screening of their child before school.
  2. If the child passes all areas of the COVID-19 screening, parent signs their child's passport for the given school day.
  3. Child brings their completed passport to the school.
  4. Teacher checks each student's passport before allowing students to enter the school.

Frequently Asked Questions


What happens if the student does not have their passport signed or forgets to bring it to school?

Students without their completed passport will be directed to the school office, and will not permitted to enter their classroom until the office has confirmed that the student has passed the COVID-19 screening for the day.


What should a student do if their COVID-19 Screening Passport is lost?

Students will directed to the office to request a new screening passport.


If a student arrives late for school, to whom do they show their passport?

Students who are late for school will show their completed passport to the office staff upon arrival and before proceeding to their classroom.
