Families and students are encouraged to review the following information:
Student Application Forms and Process
Please download the Monsignor Fraser College (MFC) School Referral Form and bring to your current or last school attended to fill it out. Please have the school send the referral form and all documents required to the campus you would like to attend.
Please complete the following application forms. Once the campus has received the referral package from the sending school, the Monsignor Fraser Campus will contact you for an interview.
Registration - What to bring to your interview appointment:
A. Proof of address
☑ Please bring a document that includes both the student’s name and current address.· If the student is 18 years or older, the document should be in their name.
· If the student is under 18 years old, the document may be in the parent’s or guardian’s name.
☑ Examples: phone bill, bank or credit card statement, utility bill, CRA document, pay stub, ODSB statement, CPP statement
⮽ Driver’s licenses and health cards cannot be accepted.
B. Proof of Status in Canada
· Born in Canada - Birth certificate / Passport
· Born outside Canada - immigration documents (Permanent Resident Card or Refugee Claimant Document)
C. Student Activity Fee
