Graduation Information 2018

Please note that Graduation Registration will only take place MPSJ on April 4-18 during both lunches in the forum. 
Registration frorms are available in the main office.  On-line option is not available.

Important Information on Immunization

Students who have received a letter from Toronto Public Health regarding outstanding immunizations are
 required to get their missing immunizations and notify Toronto Public Health that they have received them,
before April 30, 2018  (first day of active suspension period).

Students who are still outstanding on April 30, 2018 will be suspended from school for 20 days, until May 28, 2018. 

Students who are still outstanding will receive second letters from Toronto Public Health on March 1st,
and third letters on March 29th if they are still outstanding by that date.

Students who have received their immunizations and have their records, please call 416-392-1250, 
or upload the records online at tph.icon.ehealthontario.ca with a health card number.

Students who still need to get their immunizations, Toronto Public Health is offering free
vaccination clinics in Etobicoke,
North York, and Scarborough. 

To make an appointment please go to tphbookings.ca.
Students can also get outstanding immunizations at any walk-in clinic or their family doctor,
except for the meningococcal – A, C, Y, W-135 Vaccine (Menactra)
 which can only be given for free at Toronto Public Health clinics.

 Students 14 and over are able to consent for their immunizations themselves and do not need a parent or guardians permission.
Also, students without a valid health card are still able to get their immunizations done at Toronto Public Health clinics free of charge.

 For questions regarding the school immunization program, please call 416-392-1250.

 SOC schedule for education sessions:

 Sessions for are available to everyone.
For parents wishing to submit a non-medical exemption (Statement of Conscience),
attendance is mandatory. Sign up for a session:

    February 7 at Scarborough Civic Centre 2 p.m. – 3 p.m.

    March 7 at North York Civic Centre, 2 p.m. – 3 p.m.

    March 21 at Etobicoke Civic Centre, 2 p.m. – 3 p.m.

    April 4 at City Hall Council Chamber, 10 a.m.  – 11 a.m.
